Please note that this form is not for urgent issues such as temporary suspension in the case of loss or theft


Please provide the reference for deactivation, for example Account Number*, SIM Number or Phone Number for Deactivation

*If you provide your account number all services on the account will be deactivated.
Please provide an Invoice Number and the total of that invoice in order to verify access to the account. This must be an invoice for the last 3 months.
I understand once a deactivation request has been submitted it cannot be retracted and any early termination fees will be applied for a deactivation within the contract term
